What should I bring to my initial appointment?
You should bring copies of all pertinent medical records (physicians' notes, laboratory tests), and imaging results, including actual films. This information is important to help your physician understand your medical history as well as diagnose and offer treatment options. You should also bring your insurance card, referral, and pre-cert (if applicable). Prior to your visit, you will be sent a registration packet that should be filled out. Please complete and return this information prior to your visit so we can update your information in our new computer system. We recommend that you arrive at least 15 minutes before your scheduled appointment time to complete any remaining paperwork.

What do I do if I run out of my medications?
Your NYU Medical at Trinity Center physician can mail or call in prescription refills ONLY if that physician provided the original prescription form. If this is the case, please call our prescription refill number at (212) 263-9737 and leave the prescription information - name of drug, dose, how many times taken per day, and a pharmacy telephone number. We will respond to your request as soon as possible.

Yes. We offer comprehensive travel-related services including physicals, vaccinations, and prescriptions as appropriate. Your physician can also answer any questions you might have about maintaining your health while traveling oversees.

How can I get a copy of my test results and medical record?
To receive a copy of your medical records or test results, please call (212) 263-9754. You may be required to sign a consent form for release of medical information if records are to be sent to another hospital or office.

How will I know how much I owe?
Your insurance provider will send you an Explanation of Benefits (EOB) that details the amount it has paid, any non-covered or denied amounts, and the remaining balance that you are responsible for paying to NYU Medical at Trinity Center. Many health plans require patients to pay a co-payment or deductible amount. These balances will be collected at the time of registration. Review your EOB carefully, compare it to your billing statement, and call your insurance provider or customer service number on your billing statement (1-888-442-8212) right away if you have any questions or concerns.

Why did I receive a bill for a doctor I did not see?
Certain physicians help with your medical care even though you may not meet them. Commonly, these are the doctors who read your lab results, X-rays and perform anesthesia services, among others.
